Do not send your registration form to CDPH.
The completed registration form and appropriate fee must be submitted to CPS HR Consulting by the registration deadline (see test schedule).
Post marks are not accepted. Registering one month in advance is recommended.
Your registration fee may be paid by credit card, cashier's check, company check, or money order made payable to CPS HR Consulting.
